    Hi fahrulazmi. A couple of things.

    The theme appears to contain an error. When clicking on "pointsixty options" from the Appearance menu the error is produced. It is traced to this line in functions.php ...

    $theme_data = get_theme_data(ABSPATH . 'wp-content/themes/kozuka/style.css');

    Also, you should put the theme files in a theme folder. When I unzipped the package I ended up with files everywhere. :)
